2011 Federal Children Fitness Tax Credit Receipt

Please read the following information and click accept before proceeding to print the Federal Children Fitness Tax Credit Receipt.

The Federal Children Fitness Tax Credit Receipt can only  be printed once for each child. this receipt is only for children under the age of 16 (1995) who participated in one of our 2011 programs and whose payment was received in 2011.

1. From the initial screen please enter your child's information in the "PARENTS" section. Click the "Search" key to proceed to the next screen.

2. A list of programs that you have paid for will be displayed click the "Select" button to proceed to the next screen

3.On the "Receipt Management" Screen

• Click "PARENT" tab. Enter the name of the parent to be printed on the receipt.
• Click "SAVE" to continue, if name already entered click the next tab.
• Click "ELIGIBLE AMOUNT" tab. This screen lists all payments received and eligible amounts   (by program) and the totals.
• Click the "PRINT" tab. Please verify the information.  This will OPEN A NEW WINDOW WITH THE PDF DOCUMENT. PLEASE ENSURE YOUR POP UP BLOCKER IS NOT BLOCKING THIS NEW WINDOW FROM OPENING.

PRINT PDF RECEIPT OR SAVE THE PDF TO YOUR COMPUTER AND LOGOFF
