OSC League1 – Team Sign-up Form

OSC is excited to offer several great opportunities for our players during the Women’s and Men’s League1 Home Games this season!

Read below for more information on the various opportunities, and fill out the form to express interest from your team! Please note that players/team contact will have to arrive 30 minutes prior to game time to participate in the game-day opportunities. Teams will also require to have at least one team representative present at the game.

Please note you can apply for multiple games/opportunities. OSC will schedule in teams to ensure equal participation. Games marked with a * will be played at Pine Glen. Games with a ** will be played at Sixteen Mile. Games unmarked will be played at Bronte Athletic Turf.
What roles is your team interested in participating in?
Is your team available to arrive 30 minutes prior to the kick-off, as posted above in the game selection checkboxes?(Required)
Can you confirm that a team official will be present to oversee the players are the game?(Required)
